•Select this option to create a CSV file containing quarterly employee and wage information to submit.

•Unit Number is set to 00000, edit the file if needed.

•Seasonal Indicator is not marked, edit the file if you have any seasonal employees.

•Owner/Officer is not set, edit the file if needed.

•Adjustment Reason is set as 0, indicating this is an opriginal submission, edit the file if needed.

•Wages for employees marked as SUTA exempt are not included.

•If you need to include those marked as SUTA exempt, contact us via email.

•Select this option to create a CSV file for reporting your CO FAMLI wage report.
