•Print this report to obtain the UI and WT information needed for reporting to NYS.

NYS 45 ATT Efile

•Select this option to create a WFW2/SSA formatted file with the employee information, which you can upload using the NY Tax web site.

•For the UI and WT information, print the NYS Info report to paper and use those figures for the NYS-45 WebFile entry screen(s).

NOTE

•NYS includes Employee Cafeteria and Section 125 contributions as part of taxable wages for unemployment insurance.
